MIH is a proactive healthcare model where specially trained paramedics and nurses visit you at home to provide personalized care, manage chronic conditions, and help you avoid unnecessary hospital visits.
VMSC's MIH program is currently available for patients with specific chronic conditions, such as congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD), who are at high risk of hospital readmission. Your doctor at Jefferson Lansdale Hospital can refer you to our program.
Our MIH team provides comprehensive care, including health assessments, medication management, education and support, coordination with your doctors, and connection to community resources.
VMSC's MIH program is currently partnered with Jefferson Lansdale Hospital. We are actively working to expand our partnerships with other hospitals in the region to serve a wider range of patients.
